Home Safe Support Worker – Part‑time (13‑month contract)

About the role

Join Northumberland County Council’s Home Safe Support team and help people regain independence after hospital stays or short‑term placements. This 13‑month, part‑time role involves visiting service users in their own homes across Northumberland.

Key responsibilities

• Conduct initial home visits and assessments to identify support needs.
• Provide short‑term reablement care, personal care assistance, and medication support.
• Deliver low‑level equipment and therapy equipment training where required.
• Develop, implement and review individual care plans, ensuring risk assessments are completed.
• Maintain accurate, timely documentation and comply with relevant legislation.
• Manage a caseload of service users and signpost them to additional professional services.

Required profile

• NVQ Level 3 or equivalent, or willingness to work towards it.
• Valid UK driving licence and access to a car for work purposes.
• Excellent spoken English and ability to communicate clearly with service users.
• Commitment to confidentiality, dignity and respect for all individuals.
