Mental Health Social Worker

About the role

We are looking for a dedicated Mental Health Social Worker to join the Front Door team at Salford Civic. The role combines client assessments, home visits and collaborative support to deliver essential mental health services within the community.

Key responsibilities

• Conduct comprehensive mental health assessments for clients.
• Make home visits as part of the assessment and support process.
• Develop and implement individualized care plans.
• Maintain accurate records of assessments, interventions and outcomes.
• Provide crisis intervention when required.
• Participate in regular multidisciplinary team meetings.

Required profile

• Qualified Social Worker with current registration.
• Experience in mental health settings, especially assessments and home visits.
• Strong understanding of mental health issues and related social factors.
• Excellent communication and interpersonal abilities.
• Ability to work both independently and within a team.
