Nurse Practitioner

About the role

The Centre de santé communautaire Hamilton/Niagara is seeking a Nurse Practitioner to join its interdisciplinary health team in London, ON. The role focuses on delivering comprehensive patient care, coordinating with physicians and other professionals, and supporting patients across the lifespan.

Key responsibilities

• Collaborate with members of an interdisciplinary health team.
• Coordinate and evaluate patient care and progress.
• Implement and monitor interdisciplinary care plans.
• Advise physicians of any changes in patients' conditions.
• Order, perform, and interpret diagnostic and clinical tests.
• Provide end‑of‑life counseling and bereavement support.
• Advise patients on drug and alcohol rehabilitation programs.
• Ensure infection control practices.
• Assess, monitor, and refer newborns and young infants to appropriate services.
• Conduct patient interviews, physical assessments, and take medical histories.
• Inoculate and vaccinate patients.
• Provide health maintenance education.

Required profile

• Bachelor's degree.
• Licensure as a Nurse Practitioner in the province/territory of work.
• 2 to less than 3 years of relevant experience.
• Ability to work in a health‑care institution, facility or clinic.
• Strong clinical assessment and patient‑monitoring skills.
• Knowledge of infection control, newborn care, and end‑of‑life support.
