Part‑time Patient Coordinator – Maternity Cover

About the role

We are seeking a part‑time Patient Coordinator to join a leading manufacturer of specialist medical food and equipment for tube‑fed patients. The role supports the Hospital2Home (H2H) service, helping patients transition from hospital to their own homes in Sittingbourne.

Key responsibilities

• Welcome new patients or carers to the H2H service, verifying healthcare details.
• Arrange and schedule patient deliveries via telephone and email.
• Act as primary contact for healthcare professionals, internal teams and GP surgeries.
• Process chemist orders, manage prescription collections and log communications in the internal database.
• Answer and resolve patient and stakeholder inquiries promptly.

Required profile

• Patient, caring, understanding and empathetic attitude.
• Experience in a busy, fast‑paced customer service environment.
• Professional telephone manner and strong written communication.
• Competent IT skills with ability to learn new database systems quickly.
• Excellent organisational skills, able to prioritise, multitask and stay calm under pressure.
