Personal Adviser – Leaving Care

About the role
The Leaving Care Service of Manchester City Council provides statutory support to young people aged 16‑25 who are or have been looked after. The Personal Adviser will help these young people transition to independent adulthood, ensuring they have access to education, employment, health, and financial support.
Key responsibilities
- Manage a caseload and keep accurate case records, including statutory recordings.
- Contribute to Pathway Plan assessments and promote full engagement of young people.
- Undertake assessments for young people resuming education or training after age 21.
- Work with young people to ensure their views shape assessments, pathway plans, and reviews.
- Provide flexible advice, practical assistance, and support to enable successful transitions.
- Coordinate service provision and encourage use of identified support packages.
- Supervise and monitor delivery of tasks outlined in pathway plans.
- Support financial stability by advising on budgeting and facilitating appropriate financial assistance.
- Assist young people in accessing suitable accommodation.
- Collaborate with a range of agencies to deliver an integrated, holistic response.
Required profile
- Experience supporting young people aged 16‑25, particularly those leaving care.
- Understanding of statutory requirements and pathway planning for leaving care.
- Ability to manage a caseload and maintain accurate records.
