Personal Advisor (Leaving Care Services)

About the role

We are looking for a Personal Advisor to support children and young people leaving care in Liverpool. The role involves delivering effective social care services, safeguarding, and promoting the wellbeing of care leavers.

Key responsibilities

• Provide advice and practical support to care leavers on finance, accommodation, education, training, employment, and leisure activities.
• Participate in assessment, preparation, monitoring and review of Pathway Plans with a person‑centred focus.
• Promote and facilitate contact with birth families, providing supervision and assessment as required.
• Coordinate provision of services and ensure care leavers access appropriate support.
• Maintain up‑to‑date information on care leavers’ progress and wellbeing.
• Assist young people to maintain positive health and wellbeing and access specialist health services.
• Support development of confidence and decision‑making capacity.
• Identify and assess risk and need, reporting safeguarding issues in line with legislation.
• Work collaboratively with council colleagues, statutory and voluntary partners, and community groups.
• Represent the department at meetings, reviews and case conferences.
• Enter accurate case data into electronic systems such as the Integrated Care System (ICS).

Required profile

• Experience working with children, young people or vulnerable adults, preferably in a care or social services setting.
• Knowledge of national and departmental procedures and guidance for leaving care services.
• Ability to work collaboratively across agency boundaries and with external partners.
• Strong communication and interpersonal skills to build relationships with care leavers and families.
• Commitment to safeguarding and risk assessment processes.

Required skills

• Data entry and record‑keeping in electronic case management systems (e.g., ICS).
