Pharmacist - Clinical Pharmacy

About the role

The Clinical Pharmacy Pharmacist will join the interdisciplinary team at the Halifax Infirmary, reporting to the Manager of Clinical Services. The role involves delivering direct pharmaceutical care to both in‑patients and out‑patients, providing medication counseling, monitoring therapy, and supporting student pharmacists.

Key responsibilities

• Provide patient counseling and medication monitoring for in‑patients and out‑patients.
• Conduct drug utilization reviews and ensure medication safety.
• Mentor pharmacy students and assist with teaching activities.
• Liaise with community pharmacists to coordinate care.
• Deliver drug information and education to staff.
• Participate in clinical pharmacy research projects led by the Clinical Division Manager.

Required profile

• Graduate of a recognized School of Pharmacy.
• Eligible for registration with the Nova Scotia Pharmacy Regulator.
• Hospital pharmacy residency or at least two years of direct patient‑care experience in a hospital setting (preferred).
• Basic Life Support (BLS) certification.
• Strong knowledge of medication safety and therapeutics.
• Experience in evidence‑based clinical research and critical appraisal of scientific literature.
• Effective oral and written communication skills.
• Proven ability to work within a multidisciplinary healthcare team.
