Post-16 SEND Officer – Key Worker (North West)

About the role
We are seeking a Post‑16 SEND Officer to act as a Key Worker for a local authority client in the North West. The role focuses on coordinating Education, Health and Care Plans (EHCPs), supporting transition planning and ensuring young people receive appropriate support as they move towards further education, employment or adult services. The position is remote‑working and initially offered on a 3‑6‑month fixed‑term basis.
Key responsibilities
- Coordinate and review EHCPs, managing annual reviews and updates.
- Support young people through transition planning into further education, apprenticeships, supported internships or adult services.
- Provide advice and guidance to learners, parents, carers and professionals on SEND processes and statutory responsibilities.
- Act as the primary point of contact for families, colleges, training providers and partner agencies.
- Maintain accurate caseload records and ensure compliance with SEND legislation.
Required profile
- Strong knowledge of SEND legislation, including the Children and Families Act 2014, SEND Code of Practice 2015 and SEND Regulations 2014.
- Significant experience working within post‑16 SEND teams in a local authority setting.
- Proven experience managing a caseload of young people with EHCPs and coordinating annual reviews.
