Practice Nurse – Primary Care

About the role

IC24 is seeking a Practice Nurse to join its primary‑care team in Hastings. The role involves delivering high‑quality nursing care across the patient journey, from prevention and health education to chronic disease management and specialist interventions.

Key responsibilities

• Provide comprehensive nursing assessments, treatments and health‑education in line with best practice guidelines.
• Manage long‑term conditions such as asthma, diabetes and cardiovascular disease, including reviews, monitoring and personalised care plans.
• Conduct health screenings, NHS Health Checks and risk assessments to promote wellbeing.
• Deliver specialist services including cervical screening, wound care, immunisations, travel health, ear care, phlebotomy, ECGs, blood pressure and glucose monitoring, and smoking‑cessation support.
• Identify clinical risks, safeguard vulnerable patients and ensure appropriate referrals.
• Support QOF and Enhanced Services investigations, ensuring accurate coding and high‑quality outcomes.
• Supervise and mentor Healthcare Assistants and General Practice Assistants.

Required profile

• Experienced Practice Nurse with a strong background in chronic disease management, cervical screening, wound care and health promotion.
• Ability to work independently within practice protocols and multidisciplinary teams.
• Commitment to the NMC Code of Conduct and patient‑centred care.
