About the role
We are recruiting two Primary Care Health Care Support Workers to join the Community Treatment Care (CTAC) service within General Practices of the East Health and Social Care Partnership. The role is part of an integrated nursing team and the wider multidisciplinary GP team, delivering person‑centred care in a community treatment and care room setting. The position is permanent, 36 hours per week, Monday‑Friday 08:30‑17:00.
Key responsibilities
- Support delivery of quality person‑centred nursing care and defined nursing interventions to the practice population and wider locality, primarily in the community treatment and care room.
- Work collaboratively with the integrated nursing team and multidisciplinary GP team to ensure safe, caring and respectful care.
- Travel between practice sites as required, using own vehicle, to provide timely support.
Required profile
- Experience in community or general practice settings is desirable but not essential; training will be provided on the job.
- Full UK driving licence and reliable access to a vehicle are essential.
- Eligibility to work in the United Kingdom, with appropriate right‑to‑work documentation.
