About the role
Join Blackpool Council's Neighbourhood Health and Social Care Team as a Qualified Social Worker. You will be based in community health centres, working alongside multidisciplinary professionals to support individuals and carers to stay well within their local community.
Key responsibilities
- Conduct strength‑based health, care and support assessments for service users.
- Collaborate with health, housing and safeguarding teams to deliver coordinated interventions.
- Develop and implement personalised care plans that promote independence.
- Participate in multi‑agency meetings, including MASH and safeguarding forums.
- Maintain accurate case records and ensure compliance with statutory guidance.
Required profile
- Qualified Social Worker with experience in adult or community social work.
- Demonstrated ability to work effectively within multidisciplinary teams.
- Strong communication skills and a commitment to person‑centred practice.
