Reablement Officer

About the role
Southwark Council is seeking a dedicated Reablement Officer to help residents regain independence after periods of illness, crisis or housing instability. You will work directly with individuals to develop personalised plans that build confidence and support sustainable tenancy.
Key responsibilities
- Assess residents' circumstances, strengths and aspirations to create tailored reablement plans.
- Support the development of practical life skills and confidence needed for independent living.
- Assist residents transitioning from supported accommodation or facing tenancy risk.
- Collaborate with housing, health and community partners to deliver timely, person‑centred support.
- Monitor progress and adjust plans to ensure effective outcomes.
Required profile
- Experience working with adults in social care, housing or community support settings.
- Understanding of reablement principles and early‑intervention approaches.
- Strong communication and interpersonal skills.
- Ability to work collaboratively across multi‑agency teams.
- Commitment to treating people with dignity and respect.
