Reablement Worker – Short‑Term Support Service

About the role

Join Northumberland County Council's Short‑Term Support Service as a Reablement Worker. You will provide short‑term, home‑based support to adults recovering from illness or hospital discharge, helping them regain independence and avoid unnecessary admissions.

Key responsibilities

• Deliver personal care such as showering and dressing.
• Assist with medication management and meal preparation.
• Improve mobility within the home and support safe movement.
• Help with shopping, laundry and other daily living tasks.
• Encourage and support community outings and social participation.

Required profile

• A caring nature with a genuine desire to improve people’s lives.
• Ability to work flexible AM and PM shifts on a rota.
• Strong communication skills and ability to work as part of a multidisciplinary team.
• Commitment to delivering high‑quality, person‑centred care.
