Referral and Assessment Coordinator

About the role

The Referral and Assessment Coordinator provides essential administrative support to the assessment and operations teams, ensuring that referrals, assessments, and move‑in processes run smoothly. Based in Slough, this full‑time position works closely with internal and external stakeholders to maintain accurate records throughout the referral‑to‑move‑in journey.

Key responsibilities

• Act as the first point of contact for referrals, recording details accurately and passing them to assessors.
• Coordinate assessment scheduling, update calendars and share relevant information with assessors.
• Assist assessors in drafting support plans and risk assessments.
• Maintain up‑to‑date bed vacancy, hours, and service‑user records across systems and spreadsheets.
• Prepare, update and circulate service‑user documentation such as support plans, risk assessments, compatibility checks and move‑in forms.
• Liaise with care teams, operations, finance, housing providers and external partners to support funding decisions and capacity updates.
• Manage referral follow‑up, chase costings and funding decisions, and ensure timely communication.
• Ensure service‑user files are complete, audit‑ready and stored correctly.
• Produce regular reports on vacancies, capacity, service‑user hours and portal updates.

Required profile

• Minimum 3 years of office administration experience (essential).
• Previous experience in mental health settings preferred.
• A‑levels in English and Maths or equivalent.
• Relevant administration, business support or health‑social‑care qualification desirable but not essential.
