Rehabilitation Community Nurse – North West Rehabilitation Service

About the role

The North West Rehabilitation Service is seeking a Band 6 Registered Nurse to join a multidisciplinary team delivering community‑based rehabilitation for older adults and people with disabilities. You will work across the north‑west of Glasgow, providing person‑centred care in patients' homes and community settings.

Key responsibilities

• Lead clinical assessments, develop and evaluate evidence‑based care programmes.
• Manage a complex caseload of patients with multiple pathologies, promoting independence and self‑management.
• Collaborate with physiotherapists, occupational therapists, social workers and other professionals.
• Supervise support workers and nursing students, providing mentorship and professional development.
• Ensure consent, capacity assessments and culturally appropriate care involving patients, families and carers.

Required profile

• RCN‑registered Nurse, Band 6 level.
• Experience working in a multidisciplinary community team.
• Background in neurology, older‑people care or rehabilitation is desirable.
• Full UK driving licence and ability to travel across the service area.
