Rehabilitation Support Worker – Home & Community Support

About the role
We are seeking a compassionate Rehabilitation Support Worker to assist a gentleman in his early 40s with daily living, therapy activities and community engagement. Working in his home and local area, you will help him maximise independence and achieve his personal rehabilitation goals.
Key responsibilities
- Support rehabilitation programmes set by occupational and physiotherapy therapists.
- Assist with personal care and daily living tasks while promoting independence.
- Facilitate community access, social activities and participation in hobbies such as volunteering at Dogs Trust, gaming and online football management.
- Participate in therapy‑led activities and encourage fitness and mobility exercises.
Required profile
- Genuine passion for supporting others and a flexible, reliable approach to shift work.
- Strong communication and teamwork skills.
- Willingness to follow guidance and support goal‑based rehabilitation.
- Full UK driving licence.
- Enhanced DBS check (paid if required).
