Resettlement Worker - South Cluster

About the role

Hestia is seeking a Resettlement Worker for its Complex Needs Service in Hammersmith and Fulham. You will support individuals with mental health challenges, substance misuse issues and other complex needs, helping them achieve independence and rebuild their lives.

Key responsibilities

• Monitor health and safety within accommodation‑based settings and report maintenance concerns.
• Provide floating, outreach and accommodation‑based support, delivering person‑centred key working and support planning.
• Conduct risk assessments and develop recovery‑focused support packages.
• Liaise with external professionals, housing agencies and benefit services to coordinate assistance.
• Maintain accurate case records using the organisation’s case management system.

Required profile

• Self‑motivated and able to work independently as well as part of a team.
• Flexible to work evenings, weekends or bank holidays as needed.
• Willingness to learn and align with Hestia’s values of equity, diversity and inclusion.
• Ability to travel across the borough to meet service needs.

Required skills

• Basic housing management knowledge.
• Strong literacy, numeracy and general IT proficiency.
• Experience using case management systems for record‑keeping.
• Clear understanding of safeguarding principles and their application.
