Reviewing Officer – Access & Duty (Social Care)

About the role

The Reviewing Officer will join Hackney Council’s Access & Duty team to carry out strengths‑based reviews and reassessments for people with long‑term conditions. Working independently, you will visit service users in their homes or care homes, develop budgeted support plans and ensure they promote independence.

Key responsibilities

• Conduct assessments of social and functional abilities for a caseload of service users.
• Develop person‑centred, strengths‑based support plans using the Resource Allocation System.
• Monitor and update plans, including residential nursing placements, to ensure effectiveness.
• Provide information, advice and sign‑post universal services, including Telecare where appropriate.
• Maintain accurate records on client and document‑management databases and meet performance indicators.
• Gather feedback through questionnaires, complaints and surveys, and act on any shortfalls.

Required profile

• Experience working with adults who have long‑term health conditions.
• Ability to work autonomously and manage a caseload.
• Strong communication skills for interacting with service users, families and multidisciplinary teams.
• Commitment to the principles of the Care Act and personalisation.
