Senior Mental Health Practitioner – Initial Response Service

About the role

The Initial Response Service (IRS) is seeking a senior mental health practitioner to join the Fylde Coast Home Treatment Team. You will provide rapid, community‑based assessments for individuals experiencing acute mental health crises, helping to avoid unnecessary hospital admissions.

Key responsibilities

• Deliver first‑line telephone triage and assessment for service users at risk of crisis.
• Conduct comprehensive mental health and risk assessments, offering immediate care and sign‑posting.
• Refer patients to appropriate clinicians, services, or agencies as required.
• Act as a gatekeeper according to the service’s operating procedures.
• Provide managerial support to the team when the Team Manager is absent.
