Senior Social Worker – Adults

About the role
We are seeking an experienced Senior Social Worker to join the Adult Social Care team at University Hospital Coventry & Warwickshire. The role is hybrid, offering 37 hours per week at £31.97 per hour.
Key responsibilities
- Manage a complex caseload within Adult Social Care.
- Undertake assessments, care planning, and reviews in line with statutory requirements.
- Support safe and effective hospital discharges.
- Lead on safeguarding enquiries and promote best practice.
- Work collaboratively with health professionals, partner agencies, service users, and their families.
- Provide professional expertise and guidance within the team.
Required profile
- Qualified Social Worker with current Social Work England registration.
- Significant experience working within Adult Social Care.
- Proven experience in hospital discharge planning.
- Strong knowledge and practical experience of safeguarding adults.
- Ability to work effectively within a multi‑disciplinary environment.
