Senior Social Worker – Connecting Care Team (Wakefield)

About the role

Wakefield Council’s Connecting Care Team is seeking an experienced Senior Social Worker to deliver high‑quality adult social care across the borough. The role focuses on strength‑based assessments, integrated care planning and multi‑agency collaboration.

Key responsibilities

• Conduct statutory Care Act assessments, support plans and complex reviews in line with the Care Act 2014.
• Use the CareDirector system daily to record case notes, update assessments and manage workflow.
• Provide senior oversight on multi‑agency cases and mentor junior staff or student social workers.

Required profile

• Qualified social worker with active registration with Social Work England.
• Extensive experience applying the Care Act 2014 in an adult social care setting.
• Recent, hands‑on experience with the CareDirector system.
• Valid UK driving licence, access to a vehicle and business insurance for community visits.
