Social Work Support Assistant

About the role

The Social Work Support Assistant will work within the Countywide Learning Disabilities team, providing direct support to adults with care needs aged 18 and over. You will act as the main point of contact for service users and their families, helping them achieve independence and safety in daily life.

Key responsibilities

• Carry out needs and carers' assessments, incorporating safeguarding and risk considerations.
• Develop and monitor personalised care plans, reviewing continuing healthcare needs.
• Collaborate with social workers, care managers, GP practices, mental health and physical health services.
• Participate in duty system duties, discharge planning, and multidisciplinary shadowing.
• Document and retrieve case information using council IT systems.

Required profile

• Strong commitment to empowering adults with learning disabilities.
• Ability to work both independently and as part of a multidisciplinary team.
• Excellent time‑management and prioritisation skills.
• Clear communication and advocacy abilities.

Required skills

• IT proficiency for data entry and information retrieval.
