Social Worker – Adult Community Team (Full‑Time)

About the role
We are seeking an experienced, resilient Social Worker to join the Adult Community Team in South Tyneside. The role focuses on delivering strength‑based, person‑centred support to adults with complex needs, while maintaining a healthy work‑life balance within a supportive team environment.
Key responsibilities
- Manage a community caseload of adults with physical disabilities, older age, and long‑term health conditions.
- Conduct holistic Care Act (2014) assessments and develop creative support plans that promote independence.
- Lead statutory Safeguarding Adults inquiries, protecting vulnerable adults while encouraging autonomy.
- Collaborate with local health partners, occupational therapists, housing teams and other agencies to provide integrated care.
Required profile
- Registered with Social Work England as a qualified Social Worker.
- Proven experience in a statutory Adult Social Care setting (community, integrated or locality teams).
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.
- Ability to communicate warmly with service users and build rapport quickly.
