Social Worker – Assessment (North East)

About the role
We are looking for a compassionate Social Worker to join our Assessment team in the North East region. You will work within an inclusive environment, supporting individuals and families through comprehensive assessments, tailored care plans and advocacy.
Key responsibilities
- Conduct face‑to‑face assessments and home visits to identify needs, strengths and risks.
- Develop and implement personalised care plans in collaboration with clients and multidisciplinary teams.
- Provide emotional support, counselling and guidance to clients facing social challenges.
- Advocate for clients’ rights and facilitate access to community resources and services.
- Maintain accurate, confidential case records in line with organisational and legal standards.
- Collaborate with external agencies and professionals to enhance service delivery.
- Participate in team meetings, case conferences, supervision sessions and ongoing professional development.
Required profile
- Accredited Social Work qualification and registration with the relevant professional body.
- Experience in assessment and intervention within social care settings.
- Strong communication and interpersonal skills with the ability to engage empathetically with diverse populations.
- Knowledge of relevant legislation, policies and safeguarding procedures.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to diversity, equality and inclusion.
- Resilience and professionalism in managing complex situations.
