Social Worker – Learning Disabilities Team

About the role

Our client, Swindon Council, is seeking a qualified Social Worker to join its Learning Disabilities Team. The role focuses on delivering high‑quality, person‑centred services to adults with learning disabilities, promoting independence, wellbeing and community inclusion.

Key responsibilities

• Manage a caseload of adults with learning disabilities, delivering timely assessments, support plans and reviews.
• Conduct strengths‑based assessments under the Care Act 2014 and complete MCA assessments and Best Interests decisions where required.
• Undertake safeguarding enquiries in line with Section 42 of the Care Act and local procedures.
• Develop person‑centred care and support plans that maximise independence and community participation.
• Carry out risk assessments, implement risk‑management plans and promote positive risk‑taking.
• Work collaboratively with individuals, families, carers, health professionals, housing providers and voluntary organisations.
• Support transitions from Children’s Services to Adult Social Care and participate in Continuing Healthcare assessments.
• Prepare reports for funding panels, legal proceedings and statutory meetings.
• Maintain accurate case records using the electronic case management system and monitor commissioned care packages.
• Respond to urgent situations and take part in duty arrangements as needed.

Required profile

• Ability to work full‑time on a temporary basis in Swindon.
• Willingness to register with Remedy Recruitment for locum positions.
