Social Worker – Transitions Team

About the role

Our client, an East Midlands authority, is looking for experienced agency social workers to join its Transitions service. You will work with young adults moving from children’s to adult services, helping them and their families navigate this critical period.

Key responsibilities

• Carry out assessments under the Care Act and mental capacity assessments.
• Develop person‑centred transition plans and conduct care planning and reviews.
• Collaborate with children’s social workers, adult social care teams, SEND services, schools, colleges, health professionals and housing providers.
• Provide safeguarding and risk management support.
• Support young adults and their families throughout the transition process.

Required profile

• Qualified Social Worker fully registered with Social Work England.
• Significant post‑qualified experience in adult social care.
• Demonstrable knowledge or experience working with adults with disabilities or within a Transitions team.

Required skills

• Knowledge of the Care Act and mental capacity legislation.
• Experience of multi‑agency working and safeguarding procedures.
