Staff Nurse – Community Healthcare

About the role

We are seeking an experienced Staff Nurse to join our Woodsley Neighbourhood Teams in West Leeds. You will provide high‑quality, patient‑centred care in the community, supporting early discharge, reducing hospital admissions and delivering care in patients' homes.

Key responsibilities

• Deliver safe, compassionate nursing care within a multidisciplinary Neighbourhood Team.
• Collaborate with District Nurses, Community Matrons, Occupational Therapists, Physiotherapists, Clinical Assistants and other professionals to provide holistic care.
• Manage cases, coordinate care plans and support patients to self‑manage their health.
• Contribute to early discharge initiatives and help reduce unnecessary hospital admissions.
• Maintain accurate clinical records and adhere to NHS policies and guidelines.

Required profile

• Excellent communication skills with the ability to motivate and support colleagues.
• Dynamic, innovative and passionate about community nursing.
• Experience of case management and working within multidisciplinary teams.
• Resilient and able to adapt to change while maintaining high standards of care.
