Staff Nurse - Community Nursing

About the role

We are looking for a caring and motivated Registered Nurse to join our Community Nursing Team at Greenock Health Centre. The role involves delivering high-quality, person-centred care to patients in their homes and community settings across Inverclyde.

Key responsibilities

• Assess, plan, implement and evaluate individualised care programmes for community patients.
• Collaborate with multidisciplinary and multi-agency teams to ensure seamless care from admission to discharge.
• Manage a diverse caseload, making clinical decisions and solving problems effectively.
• Promote health and wellbeing through education, support and preventative interventions.
• Maintain accurate clinical records and meet professional documentation standards.
• Support and mentor students, newly qualified staff and support workers.
• Participate in audit, quality improvement and clinical governance activities.

Required profile

• Registered Nurse with a current NMC UK PIN.
• Experience in community nursing or a willingness to develop in this area.
• Ability to work independently and as part of a multidisciplinary team.
• Commitment to delivering person-centred care and promoting patient independence.
