Staff Nurse – Supported Discharge Unit

About the role

The Supported Discharge Unit at Victoria Hospital is seeking an enthusiastic registered nurse to join a fast‑paced, nurse‑ and doctor‑led team. This full‑time position offers the chance to develop both clinical and leadership skills while working across day, night and weekend shifts.

Key responsibilities

• Deliver safe, effective, patient‑centred care in a high‑volume acute setting.
• Collaborate with physiotherapists, occupational therapists, social workers and the Health and Social Care Partnership to coordinate discharges.
• Facilitate transfers of care to community settings and support patients returning home.
• Contribute to the development and improvement of care pathways.
• Maintain high standards of documentation and uphold organisational objectives.

Required profile

• Registered Nurse with a commitment to patient‑centred care.
• Excellent communication and teamwork abilities.
• Post‑registration experience desirable.
• Understanding of acute medical and chronic illness complexities.
• Willingness to join the Protection of Vulnerable Groups (PVG) Scheme and meet UK work‑eligibility requirements.
