Support Coordinator – Stroke Services (Part‑time)

About the role

We are seeking an enthusiastic and motivated Support Coordinator to join our South West London team, reporting to Service Delivery Coach Gavin Hughes. The role is home‑based, part‑time (28 hours per week) with regular travel to meet stroke survivors and their carers.

Key responsibilities

• Organise and facilitate effective communication for stroke survivors and carers to help them understand and achieve their goals.
• Maintain timely, confidential, and accurate records in our CRM system, ensuring compliance with retention policies and GDPR.
• Develop and manage volunteers to support service delivery for stroke survivors and carers.

Required profile

• Experience providing person‑centred support to vulnerable individuals or those with disabilities or long‑term health conditions and their carers.
• Emotional resilience to handle complex and challenging situations, often from a home office.
• Strong listening and communication skills with diverse audiences, including those with communication difficulties, using face‑to‑face visits, phone, email, or video calls.
• Ability to travel across a large geographical area as required.
