Support Worker – Care for Individuals with Complex Needs

About the role

Urban Care Community is seeking compassionate, self‑employed Support Workers to provide personalised care for adults with learning disabilities, autism, mental health conditions and other complex needs. You will work directly with clients in their homes or community settings, helping them achieve greater independence and wellbeing.

Key responsibilities

• Deliver direct personal care and support tailored to each individual’s needs.
• Develop and implement personalised care plans in collaboration with clients, families and professionals.
• Administer and monitor medication according to prescribed protocols, maintaining accurate records.
• Apply behaviour‑management strategies to reduce challenging behaviours and promote positive outcomes.
• Maintain a safe, inclusive environment that supports physical, emotional and social wellbeing.
• Work closely with multidisciplinary teams, including medical staff, therapists and educators.
• Complete and update documentation such as progress notes, incident reports and behaviour charts.

Required profile

• Minimum two years’ experience supporting individuals with learning disabilities, autism, mental health or related conditions.
• Strong knowledge of best practices in complex‑need care, including medication administration and crisis intervention.
• Excellent interpersonal and communication skills for building rapport with clients and families.
• Ability to work independently while adhering to regulatory standards.
