Support Worker – Home & Community Care

About the role

We are seeking a dedicated Support Worker to join our established team delivering personalised care and rehabilitation for a young disabled man in his own home and within the community. The role requires flexibility to cover daytime, night and weekend shifts.

Key responsibilities

• Provide personal care and assistance with daily living activities.
• Support the client’s rehabilitation programme and work alongside senior therapy staff.
• Accompany the client on community outings and ensure safety at all times.
• Follow the agreed care plan and record observations accurately.
• Work a rotating roster that includes day, night and weekend shifts.

Required profile

• Minimum 1 year experience in community care or support work.
• Experience in a rehabilitation environment is an advantage.
• Flexible approach with availability for varied shifts, including nights and weekends.
• Excellent communication skills and a positive, friendly attitude.
• Valid UK driving licence and eligibility for a fully funded DBS check.
