Support Worker – Young Adults Leaving Care

About the role

Elliot Leigh Supported Living is seeking a dedicated Support Worker to help 18‑25‑year‑old care leavers transition to independent living. You will work directly with residents, building practical life skills and providing emotional support during a pivotal stage of their lives.

Key responsibilities

• Assist residents with budgeting, cooking, cleaning and tenancy management.
• Deliver structured key‑work sessions and co‑create individual support plans.
• Support education, employment and training goals, including CV writing and job applications.
• Maintain accurate digital case records, risk assessments and support plans.
• Collaborate with the wider team to ensure resident safety and wellbeing.
• Liaise with local authority leaving‑care teams, social workers and other professionals.
• Provide trauma‑informed support for mental health and personal challenges.
• Accompany residents to external appointments (GP, Job Centre, etc.).

Required profile

• At least one year of experience working with young adults (18‑25) in care, supported housing or semi‑independent settings.
• Understanding of the challenges faced by care leavers.
• Knowledge of safeguarding legislation and trauma‑informed practice.
• Ability to work flexibly across evenings, weekends and bank holidays.
• Calm, professional response to conflict, emotional distress or challenging behaviour.
