Team Leader – Extra Care Service

About the role

We are seeking a dynamic, enthusiastic and highly organised Team Leader to join our Extra Care service in Leeds. Working alongside the Registered Manager, you will coordinate person‑centred support for older adults, both in their own flats and in communal facilities.

Key responsibilities

• Deliver direct care and oversee staff deployment across the service.
• Supervise and coach staff, providing informal and formal supervision and conducting regular appraisals.
• Assess prospective tenants’ care and support needs and develop personalised support plans.
• Maintain communication with families, promoting their involvement in care planning and activities.
• Support tenants to develop personal and life skills, encouraging participation in social, leisure and recreational activities.
• Collaborate with families, external agencies and other professionals to ensure a holistic approach to care.

Required profile

• Minimum three years’ experience in a care or health‑social‑care setting.
• Relevant qualifications (e.g., QCF Diploma in Health and Social Care or equivalent).
• Excellent communication skills and a strong sense of duty of care.
• Commitment to person‑centred support and promoting independence, choice and wellbeing.
