Team Leader – Substance Misuse Service

About the role

We are seeking an experienced Team Leader to join our community‑based treatment service for adults with drug and/or alcohol difficulties. The role combines clinical expertise, therapeutic delivery and leadership of a multidisciplinary team.

Key responsibilities

• Maintain a small caseload while supporting practitioners to deliver high‑quality assessments, SMART care plans and structured interventions.
• Provide harm‑reduction advice, conduct urine and oral swab testing, and oversee group work and treatment programmes.
• Lead the Rough Sleepers Initiative, managing frontline staff and coordinating with partner agencies such as GPs, hospitals, housing providers and probation.
• Develop and sustain effective partnerships to ensure seamless service delivery for people experiencing homelessness and substance misuse.
• Monitor outcomes, ensure compliance with CQC standards and meet KPIs set by commissioners and The Forward Trust.
• Work flexibly, including occasional evenings or weekends, to meet service demands.

Required profile

• Proven experience managing or leading within a community‑based substance misuse service.
• Skilled in comprehensive assessments and designing high‑quality SMART care plans.
• Ability to deliver structured one‑to‑one and group interventions and mentor staff.
• Strong knowledge of psychosocial interventions and motivational interviewing techniques.
• Confidence using IT systems for case management, record keeping and service delivery.
