Team Manager – Supported Living (Hammersmith & Fulham)

About the role

Certitude is seeking a Team Manager to lead a dedicated team providing supported living care in Hammersmith & Fulham. You will oversee a permanent team of ten support staff, ensuring high‑quality, person‑centred services for adults with learning disabilities or mental health needs.

Key responsibilities

• Lead, coach and train the team in person‑centred support practices.
• Conduct regular one‑to‑one meetings, team briefings and annual performance reviews.
• Manage risk by ensuring staff understand individual support priorities.
• Complete administrative tasks, documentation and detailed written reports using IT systems.

Required profile

• Experience supporting people with learning disabilities or mental ill health.
• Proven ability to enable choice and control for service users.
• Track record of leading teams to embed person‑centred working.
• Understanding of challenges faced by people with disabilities or mental health conditions.
