Adults Social Worker – Hospital Discharge

About the role

Our client, Rochdale Borough Council, is seeking an experienced Adults Social Worker to join their Hospital Discharge team. You will work across multiple hospital sites, supporting adults and their carers as they transition from hospital to community care.

Key responsibilities

• Develop and maintain positive relationships with health colleagues across all local hospital sites.
• Carry out Mental Capacity Assessments and Best Interest Processes in line with the Mental Capacity Act.
• Plan and coordinate discharge arrangements, promoting a strength‑based approach and meeting Care Act 2014 objectives.
• Utilise natural support networks and the voluntary sector to enhance independence and well‑being.
• Maintain accurate records using both manual and electronic systems.
• Visit hospital sites as required and respond to daily service demands, with some home‑working flexibility.

Required profile

• Qualified social worker with current hospital social work experience.
• Strong knowledge of the Mental Capacity Act and Care Act 2014.
• Excellent organisational, interpersonal and communication skills.
• Ability to work flexibly, manage multiple priorities and meet tight deadlines.
• Motivated, forward‑thinking professional who works well as part of a team.
• Access to a car for site visits.

Required skills

• Proficiency in Microsoft Word.
