Adults Social Worker – Learning Disabilities & Autism

About the role

Our client, Telford and Wrekin Council, is seeking a qualified Adults Social Worker to join the Learning Disabilities and Autism team. You will manage a caseload of adults, delivering strengths‑based assessments, support plans and safeguarding interventions to promote independence and wellbeing.

Key responsibilities

• Maintain a professional caseload, completing assessments, support plans, reviews and risk assessments.
• Apply strengths‑based and asset‑based approaches with individuals and families.
• Promote safeguarding and undertake investigations where required.
• Collaborate with statutory, voluntary and community partners to achieve agreed outcomes.
• Participate in peer reviews, reflective practice sessions and team discussions.
• Maintain accurate case records, reports and documentation.
• Provide advice and guidance to colleagues, support less experienced staff and students.
• Undertake duty responsibilities as part of the team rota.

Required profile

• Qualified Social Worker registered with Social Work England.
• Knowledge of Adult Social Care legislation, including the Care Act 2014 and Mental Capacity Act 2005.
• Ability to make evidence‑based decisions and manage risk effectively.
• Commitment to continuous professional development and reflective practice.
• Strong partnership‑working and communication skills.
