Adults Social Worker – Learning Disabilities

About the role
This position is with Somerset Council, joining the Learning Disabilities team to deliver high‑quality Care Act reviews and reassessments for adults with learning disabilities across the South West.
Key responsibilities
- Undertake detailed Care Act reviews and reassessments as part of the Review Project.
- Conduct face‑to‑face visits in homes, supported living, residential settings and other community environments to gather evidence.
- Analyse information from service users, families, advocates, providers, health professionals and records to inform professional decisions.
- Determine the need for a statutory review or a full reassessment, providing clear professional rationale.
- Identify and record eligible care and support needs in line with the Care Act, using strengths‑based, person‑centred practice.
- Produce audit‑ready assessments, support plans and case recordings that demonstrate professional judgement.
- Distinguish between social care needs, delegated health tasks, NHS responsibilities and health‑related needs requiring further consideration.
- Identify cases for Continuing Healthcare (CHC) or Checklist review and provide evidence‑based recommendations.
- Participate in multidisciplinary discussions, funding reviews and CHC processes.
- Manage an allocated workload independently, meeting project timescales and organisational expectations.
- Collaborate with adult social care teams, health colleagues, providers and other stakeholders to achieve timely, defensible outcomes.
- Attend Somerset office locations (minimum two days per week) and travel across Somerset for visits and project activities.
Required profile
- Qualified social worker with experience in adult learning disabilities.
- Strong knowledge of the Care Act and Continuing Healthcare processes.
- Ability to work independently, manage workload and meet deadlines.
- Excellent analytical and decision‑making skills, with a person‑centred approach.
- Effective communication and collaboration with multi‑disciplinary teams.
