Adults Social Worker – Long Term Care Service

About the role

The Adults Social Worker will join Vale of Glamorgan Council’s Long Term Care Service in Barry. You will assess, plan and commission support for adults with care needs, working closely with service users, carers and partner organisations.

Key responsibilities

• Undertake statutory assessments and develop personalised care plans.
• Plan, commission and monitor services to help adults and carers achieve their outcomes.
• Manage a caseload, prioritising workload and maintaining up‑to‑date electronic case files.
• Coordinate resources and collaborate with internal and external statutory and voluntary partners.
• Participate in supervision, personal development reviews and ongoing training.
• Mentor or supervise junior staff or students where appropriate.
• Contribute to multi‑disciplinary team duties, office rotas and compliance with council policies.

Required profile

• Registration with Social Care Wales and willingness to maintain it.
• Commitment to continuous professional development, including potential training as an Approved Mental Health Practitioner.
• Ability to work collaboratively with service users, carers and a range of partners.
• Strong organisational skills to manage caseloads and documentation.
