Advanced Clinical Pharmacist – Primary Care (Aberdeen)

About the role
The Primary Care Advanced Clinical Pharmacist will join the Aberdeen City Health and Social Care Partnership to lead and develop pharmacotherapy services across GP practices and the pharmacy hub. This full‑time position (part‑time possible) focuses on safe, appropriate, and cost‑effective medicines management and provides high‑quality pharmaceutical care to patients.
Key responsibilities
- Conduct medication and polypharmacy reviews.
- Perform medicines reconciliation and action hospital discharge letters.
- Manage acute and repeat prescription requests.
- Provide prescribing advice to multidisciplinary teams.
- Respond to ad‑hoc clinical queries and participate in MDT meetings.
- Support social‑care colleagues with medication‑related issues, including care‑at‑home services.
- Supervise, train and mentor pharmacy team members.
Required profile
- MPharm or degree in Pharmacy.
- Registered with the General Pharmaceutical Council (GPhC).
- Independent Prescriber qualification.
- Completion of a clinical diploma or Advanced Competency Framework, or equivalent experience.
- Strong IT skills and excellent communication.
- Flexibility to work across Aberdeen City.
- Full driving licence and access to a vehicle.
