Advanced Clinical Practitioner – Home Visiting Service

About the role

The Advanced Clinical Practitioner (ACP) will join IC24’s East Sussex Home Visiting Service, delivering high‑quality clinical assessments and treatments to patients in their own homes. Working autonomously within a multidisciplinary team, the ACP will manage acute, urgent and undifferentiated presentations, helping to reduce unnecessary hospital admissions.

Key responsibilities

• Assess, diagnose and manage patients in the community, providing timely and evidence‑based care.
• Prescribe medication as an Independent Prescriber following clinical assessment.
• Make safe clinical decisions, arrange investigations, referrals and follow‑up plans.
• Collaborate with GPs, nurses, NHS 111, allied health professionals and secondary care partners to ensure continuity of care.
• Participate in audits, clinical governance and quality‑improvement initiatives.
• Maintain professional development and contribute to mentoring and shared learning.

Required profile

• Qualified health professional with extensive clinical experience.
• Registered Independent Prescriber with full prescribing rights.
• Ability to work autonomously and make sound clinical judgments.
• Strong communication skills for multidisciplinary collaboration.
