Band 5 Occupational Therapist – Huddersfield

About the role

Cygnet Health Care is seeking a Band 5 Occupational Therapist to join its enhanced community residential services at Gledholt, Gledholt Mews and Coach House in Huddersfield. You will work within a nurse‑led, multidisciplinary team to support adults with learning disabilities or mental health needs who present with challenging behaviours.

Key responsibilities

• Conduct comprehensive occupational therapy assessments for residents.
• Develop and implement person‑centred care plans, including the Visual Discharge Plan (VDP) approach.
• Collaborate with psychologists, nurses, speech and language therapists, and activity coordinators to deliver integrated care.
• Monitor progress, adjust interventions, and support residents towards greater independence.
• Participate in MDT meetings, case reviews, and discharge planning from admission.

Required profile

• Qualified Occupational Therapist (HCPC‑registered) or final‑year OT student about to qualify.
• Experience or strong interest in working with adults with learning disabilities or mental health conditions.
• Ability to work in a nurse‑led environment and follow clinical governance.
• Commitment to 40 hours per week on‑site.
