Band 5 Practice Nurse

About the role

The Band 5 Practice Nurse will be based at Halton Medical Centre, supporting primary‑care services for the Royal Air Force community and local population. You will deliver high‑quality nursing care, health promotion and disease prevention within an NHS environment.

Key responsibilities

• Conduct comprehensive patient assessments and develop care plans.
• Provide immunisations, wound care, and routine health checks.
• Maintain accurate clinical records and contribute to multidisciplinary team meetings.
• Promote health education and preventive strategies to patients and staff.
• Adhere to NHS policies, infection control standards and MoD security requirements.

Required profile

• Registered Nurse with a current NMC registration.
• Eligibility to work for the Ministry of Defence and undergo security checks.
• Demonstrated ability to work autonomously and as part of a team.
• Commitment to continuous professional development.

Required skills

• Clinical assessment and patient management.
• Effective communication with diverse patient groups.
• Proficiency in using NHS electronic health record systems.
