Band 6 Clinical Triage Practitioner

About the role

We are seeking three experienced Band 6 Clinical Triage Practitioners to join a multidisciplinary team serving the Bath and North East Somerset, Swindon and Wiltshire area. In this fast‑paced role you will manage referrals and ensure patients are directed to the most appropriate community‑based care pathway.

Key responsibilities

• Assess incoming referrals from healthcare professionals and determine the correct care pathway.
• Provide clinical triage for services such as Hospital at Home, Integrated Neighbourhood Teams, Urgent Care Response and other community services.
• Work through waiting‑list referrals to guarantee timely patient access.
• Collaborate with a multidisciplinary team across multiple sites.
• Follow shift patterns covering 8 am‑8 pm, 8 am‑4 pm or 12 pm‑8 pm as required.

Required profile

• Registered Nurse with Band 6 experience.
• Previous clinical triage experience in a multidisciplinary setting.
• Valid NHS Smartcard.
• Ability to make sound clinical judgments and work effectively within a team.

Required skills

• SystmOne (clinical IT system).
