Band 6 Community Psychiatric Nurse (CPN) – Southend

About the role

We are seeking an experienced Band 6 Community Psychiatric Nurse to join the Recovery & Wellbeing Team at the Taylor Centre in Southend. The role involves caring for patients with severe and enduring mental health conditions within a community setting.

Key responsibilities

• Manage a caseload of up to 35 patients with severe and enduring mental health conditions.
• Act as Care Coordinator in line with the Care Programme Approach (CPA).
• Complete comprehensive mental health and risk assessments.
• Develop, implement, and review person‑centred care plans.
• Collaborate with multidisciplinary teams and external agencies.
• Provide therapeutic interventions and ongoing support to service users.
• Maintain accurate clinical records and documentation according to NHS policies.
• Promote recovery, independence, and positive mental health outcomes.

Required profile

• Valid NMC registration as a Registered Mental Health Nurse.
• Previous experience as a Community Psychiatric Nurse (CPN).
• Experience managing a community mental health caseload.
• Knowledge of the CPA, risk assessment, and safeguarding procedures.
• Up‑to‑date mandatory training.
• Enhanced DBS clearance.
• Availability to start as soon as possible.
