Care Coordinator

About the role
The Care Coordinator ensures residents receive high‑quality, person‑centred care within our Walton‑on‑Thames care home. Working closely with the care team, you will organise shifts, manage medication, and liaise with health professionals to support each resident’s individual needs.
Key responsibilities
- Coordinate and manage daily care plans for residents.
- Organise staff shifts and ensure appropriate coverage.
- Administer and accurately record medication.
- Liaise with external health professionals and update computerized care plans.
- Assist with induction, training and supervision of support staff.
Required profile
- Experience supervising a care team.
- Empathetic understanding of resident needs.
- Strong leadership and communication abilities.
- Attention to detail, organised and methodical work style.
- Flexibility regarding schedules, roles and responsibilities.
- Working towards or holding an NVQ Level 3 in Health & Social Care.
