Care Coordinator

About the role

An experienced Care Coordinator is needed to support a well‑established domiciliary care provider in Eastbourne. Working closely with the Registered Manager, you will help ensure safe, high‑quality, person‑centred care for clients while managing staff rotas and care packages.

Key responsibilities

• Support the Registered Manager with day‑to‑day service coordination.
• Manage staff and client rotas to guarantee effective care delivery.
• Complete client assessments and develop person‑centred care plans.
• Liaise with clients, families, care staff and healthcare professionals.
• Ensure care complies with CQC regulations.
• Participate in the on‑call rota and provide community support when required.

Required profile

• Previous experience as a Care Coordinator or similar role in domiciliary care.
• Good understanding of CQC regulations and person‑centred care planning.
• Experience completing client assessments and care plans.
• Strong communication, organisational and IT skills.
• Willingness to undertake care visits when needed.
