Care Coordinator

About the role

We are seeking a motivated Care Coordinator to lead our domiciliary service in Newport, Isle of Wight. The role focuses on supporting elderly clients, including those with Alzheimer’s and dementia, by ensuring high‑quality, person‑centred care.

Key responsibilities

• Motivate and inspire care teams through training, coaching and leading by example.
• Oversee daily service operations, including rostering, support‑plan development, staff deployment and budget management.
• Conduct introductory live‑in care visits and accompany new Live‑In Social Care Workers on their first home visits.
• Carry out risk assessments and individual needs assessments, updating them as required for compliance.
• Collaborate with multi‑disciplinary health and social teams to maximise care outcomes.
• Implement and maintain policies, procedures and legislative standards related to service delivery.

• Strong communicator able to influence and get the best from the team.
• Proven leadership and team‑management experience.
• Effective coach and mentor with a commitment to achieving positive outcomes.
• Ability to think quickly and see tasks through to completion.
