Care Coordinator – New Milton

About the role

We are seeking a proactive Care Coordinator to join our New Milton branch. In this full-time position you will organise care schedules, match carers with clients and ensure smooth delivery of services.

Key responsibilities

• Coordinate care services and client schedules.
• Match carers with clients to meet individual needs.
• Support the setup of new care packages.
• Plan rotas to ensure continuity of care.
• Communicate with families and healthcare professionals.
• Record staff availability, absences and emergency calls.
• Participate in the on-call rota and cover the on-call phone when required.

Required profile

• Strong organisational skills and attention to detail.
• Ability to work confidently in a busy, fast-paced environment.
• Minimum 1 year experience in care or a Level 2 NVQ in Health and Social Care.
• Full UK driving licence and access to your own vehicle.
• At least 12 months remaining on your right to work in the UK.
